Manufacturing Statement

In accordance with requirements of the scheme rules which govern your Certification, Certificate Holders must ensure a Certified Product materially is same as any sample that was originally tested and subsequently evaluated by CMI prior to certification.

Certificate Holders must notify CMI of any intended or actual change, modification or alteration to the Certified Product, its location or method of manufacture, Product Quality Plan or Installation Instructions.

Your completion of the Manufacturing Statement below will verify if there have been any such changes or that there have been no changes to the previously supplied Manufacturing Declaration.

Marking Declaration

It is important to ensure that the Certified Product marking and logo usage requirements of the Scheme rules are complied with. The Marking Declaration below is used to revalidate your Certified Product marking and logo usage under the relevant section of the scheme rules.

It is the responsibility of the person completing the Marking and Manufacturing Statement of Compliance below to ensure that all statements are true and correct.

If any identified changes from the previously supplied Manufacturing or Marking Declarations have occurred, please supply to CMI the updated Declaration. The completion of this Marking and Manufacturing Statement of Compliance will suffice if no changes are to be declared.
